Au moment o\u00f9 le DSM 5 est publi\u00e9 en fran\u00e7ais nous tenons \u00e0 r\u00e9affirmer notre opposition radicale aux fondements et \u00e0 l\u2019utilisation de ce manuel.Depuis plus de trente ans, le DSM a impos\u00e9 sa domination sur la psychiatrie mondiale. Con\u00e7u comme un instrument statistique pour la recherche \u00e9pid\u00e9miologique et pharmacologique, il a, petit \u00e0 petit, envahi l\u2019ensemble des domaines de la psychiatrie et, en particulier, l\u2019enseignement aux diff\u00e9rents acteurs de la sant\u00e9 mentale, ainsi que la pratique clinique.Se voulant un instrument de renouvellement et de modernisation de la d\u00e9marche diagnostique et de sa fiabilit\u00e9, il a \u00e9chou\u00e9 : les diagnostics qu\u2019il r\u00e9pertorie ne sont ni fiables, ni valides, comme le prouvent la g\u00e9n\u00e9ralisation des comorbidit\u00e9s.<\/p>\n

Qui plus est, ils ne sont pas vraiment utiles pour la recherche scientifique. Le DSM a contribu\u00e9 \u00e0 d\u00e9truire les bases de la clinique traditionnelle au nom d\u2019un espoir dans l\u2019arriv\u00e9e prochaine de marqueurs biologiques, qui ne sont pas au rendez vous. Il a ainsi, en soutenant cette croyance, fait le lit du pire r\u00e9ductionnisme scientiste en privil\u00e9giant le mod\u00e8le biologique et m\u00e9dical, au d\u00e9triment de l\u2019environnement social et de la r\u00e9alit\u00e9 psychique. Sa d\u00e9marche, fond\u00e9e sur une mise en coupe r\u00e9gl\u00e9e, comportementale, des troubles mentaux, a brouill\u00e9 la ligne de partage entre le normal et le pathologique, entra\u00eenant des fausse \u00e9pid\u00e9mies, l\u2019invention de chim\u00e8res, une surpathologisation des \u00e9motions et des comportements, jusqu\u2019aux exc\u00e8s qui font partie de la vie, avec des surdiagnostics, en particulier chez les enfants.<\/p>\n

En isolant les troubles mentaux de leur contexte d \u2018apparition, il en a fait des cibles privil\u00e9gi\u00e9es pour les m\u00e9dicaments et a favoris\u00e9 la surprescription en abaissant les seuils d\u2019inclusion.Le DSM, qui n\u2019a aucun fondement scientifique solide, s\u2019est impos\u00e9 n\u00e9anmoins comme instrument de r\u00e9f\u00e9rence de l\u2019\u00e9conomie de la sant\u00e9 et des pratiques d\u2019\u00e9valuation des administrations sanitaires. Il a permis le d\u00e9veloppement d\u2019une pens\u00e9e unique, d\u2019une novlangue, ruinant les conditions d\u2019un d\u00e9bat scientifique honn\u00eate dans le champ de la sant\u00e9 mentale d\u2019autant que les conflits d\u2019int\u00e9r\u00eats qui ont \u00e9maill\u00e9 son histoire, ont cr\u00e9\u00e9 une grave crise de confiance, de l\u00e9gitimit\u00e9 et de fiabilit\u00e9 au sein de la psychiatrie mondiale.<\/p>\n

Pour toutes ces raisons cliniques, \u00e9thiques, scientifiques et de sant\u00e9 publique, nous appelons \u00e0 r\u00e9cuser la r\u00e9f\u00e9rence au DSM 5, \u00e0 utiliser pr\u00e9f\u00e9rentiellement la CFTMEA (la Classification fran\u00e7aise des Troubles Mentaux de l\u2019Enfant et de L\u2019Adolescent) et la future CFTMA(classification Fran\u00e7aise des Troubles Mentaux de l\u2019Adulte) qui va para\u00eetre \u00e0 la fin de l\u2019ann\u00e9e 2015 , et \u00e0 ouvrir un large d\u00e9bat sur les classifications.<\/p>\n

OFFICIAL STATEMENT of The Collective Initiative for the Clinic of the Subject STOP DSM:
\nOn the occasion of the publication of the French version of the DSM 5 (fifth edition of the Diagnostic and Statistical Manual of Mental Disorders), we wish to reaffirm our radical opposition to the foundations and use of the Manual. For more than thirty years, DSM has dominated the world\u2019s psychiatric community. Originally a statistical tool intended to serve epidemiological and pharmacological research, it progressively invaded the entire field of psychiatry and especially its teaching to the different mental health actors, as well as its clinical practice. The DSM has failed in its effort to renew and modernize the diagnostic procedure and its reliability: the diagnostic categories listed in it are neither reliable nor valid, as the generalization of comorbid disorders clearly shows. Moreover, their usefulness for scientific research is equally dubious. The DSM has helped destroy the foundations of traditional clinical psychiatry, in the name of a hope for a soon-to-come discovery of biological markers, a hope that has failed to materialize. In supporting this belief, it has created a fertile ground for the worst kind of scientific reductionism, favoring the biological and medical model over the social environment and psychic reality. Its approach, based on the systematic behaviorist exploitation of mental disorders, has blurred the lines between the normal and the pathological, giving rise to false epidemics and chimeras, encouraging the over-pathologizing of emotions and behavior, including the extremes that are part of human life, as well over-diagnosis, especially concerning children. Separated from the context in which they manifest, mental disorders have become the priority targets of medication, leading to over-prescription by lowering the inclusion thresholds. The DSM, which lacks any kind of solid scientific basis, has nevertheless become the reference tool for the entire healthcare economy and the assessment methods used by healthcare authorities. It has encouraged the development of a uniform way of thinking and a kind of newspeak, destroying the conditions of a honest scientific debate in the field of mental health; because the numerous conflicts of interests in its history, it has also created a severe crisis of trust, of legitimacy and reliability within the world\u2019s psychiatric community. For all these clinical, ethical, scientific and public health reasons, we call for a rejection of the DSM as a reference. Instead, we encourage clinicians to refer to the CFTMEA (The French Classification of Child and Adolescent Mental Disorders), as well as the future CFTMA (French Classification of Adult Mental Disorders), which will be issued in late 2015, and open up a wide public debate on the questions surrounding these classifications.<\/p>\n","protected":false},"excerpt":{"rendered":"
